Strabismus is a misalignment of the eyes β€” one may turn inward, outward, up, or down. It affects about 2 to 4 percent of children and can also appear in adults. Treatment depends on the type, age, and cause.

First line

Glasses, sometimes with prism

Many cases β€” especially accommodative esotropia in farsighted children β€” improve dramatically with the right glasses. Prism built into the lens can compensate for small angles.

If amblyopia

Patching

When strabismus has caused amblyopia, treating the weak eye runs in parallel with addressing the alignment.

Adjunctive

Vision therapy

Office-based exercises can help convergence insufficiency and selected intermittent strabismus patterns, especially in older children and adults.

Surgical

Eye muscle surgery

Strabismus surgery loosens or tightens specific muscles to realign the eyes. Studies show success rates around 70-80% for horizontal strabismus, with some patients needing additional procedures.

Same-week or sooner if

An adult develops new double vision or eye misalignment β€” especially with a droopy lid, large pupil, or severe headache. These can signal cranial nerve palsy, aneurysm, or stroke. A child whose eyes drift suddenly or after a fever should also be seen promptly.

Will my baby outgrow it?+

True congenital esotropia rarely resolves on its own and benefits from early surgical alignment. Intermittent inward drifting in the first few months of life is sometimes a normal developmental phase, but a persistent crossed eye after 4 months should be evaluated.

Is strabismus surgery cosmetic?+

It's functional. Eye-muscle surgery realigns the eyes to restore or maintain binocular vision, improve depth perception, and prevent or treat amblyopia. Better appearance is a beneficial side effect.

How successful is surgery?+

Real-world studies show roughly 70-80% of patients reach good alignment after one surgery. Some patients need a second procedure as alignment can drift over time, especially in intermittent exotropia.

Can adults with longstanding strabismus benefit from surgery?+

Yes. Surgery in adults can improve eye contact, social comfort, and in some cases restore enough binocular function to relieve symptoms. Coverage by insurance is often determined by functional impact.

Does screen time cause strabismus?+

There's no good evidence that screen use causes strabismus. Excessive near work can fatigue convergence and unmask convergence insufficiency, but it doesn't create true strabismus.
